Medication
Baltimore Lutheran abides by the Baltimore County Department of Education medication policy. Any medication should be brought to school by a parent/guardian or responsible adult. The medication container must be labeled by the pharmacy with the student’s name, prescriber’s name, name of medication, dosage, route, condition for storage, prescription date, and expiration date. All over the counter medication must come in the original container. The parent must also give their written consent to give medication. You can obtain the medication form from the Nurse or it can be downloaded from the BLS website.
Our fax number is 410 825-2506.
Sports Physical
All students who wish to participate in extracurricular team activities must have a completed, current sports physical on file with the nurse’s office before he/she can attend the first practice. The sports physical form can be downloaded here, picked up in the main office, the nurse’s office or the athletic director’s office. Sports Physicals must be received in our office by 8:15 a.m. in order for the student to be cleared for play that day.
Athletic Trainer
Athletic trainers are provided by Towson Sports Medicine to cover all home athletic events including practices. The trainer will keep a record of all students seen and give further recommendations for treatment or follow-up care.
