Medication

Baltimore Lutheran abides by the Baltimore County Department of Education medication policy which has very specific guidelines for the administration of medications in school. Any medication should be brought to school by a parent/guardian or responsible adult and must be accompanied by a physician’s order on the Maryland State School Medication Administration Authorization Form. The parent must also sign the medication form giving their written consent to give medication at school. This includes prescription medication and over the counter medication i.e.: Tylenol, Motrin, etc. Prescription medication must come in a pharmacy prepared labeled bottle with the student’s name, prescriber’s name, medication name, dosage, route, prescription date and expiration date. Over the counter medication must come in the original container. The Maryland State School Medication Administration Authorization Form can be obtained from the nurse, the main office or downloaded here.

All medication must be kept in the health suite unless approved by a physician for the student to self carry i.e.: inhaler, Epi Pen.

The fax number for the health suite is (410) 583-7054.

Sports Physical

All students who wish to participate in extracurricular team activities must have a completed, current sports physical on file with the nurse’s office before he/she can attend the first practice. The sports physical form can be downloaded here, picked up in the main office, the nurse’s office or the athletic director’s office.

Athletic Trainer

Athletic trainers are provided by Towson Sports Medicine to cover all home athletic events including practices. The trainer will keep a record of all students seen and give further recommendations for treatment or follow-up care.
